What You'll Be Doing:
Provide Personalized Care: Evaluate and address the needs of individuals in their home environments, focusing on safety and productivity.
Enhance Daily Living: Assist with home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, and dementia care.
Empower Caregivers: Provide training and support to family caregivers.
Collaborate: Partner with case managers and the interdisciplinary team to deliver comprehensive care.
Travel: Provide services to patients within the Placerville area.

What You'll Bring to the Table:
Current unrestricted California Occupational Therapist License.
Current BLS/CPR Certification.
Valid California Driver's License.
Knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home healthcare (highly preferred).
Experience with high acuity patients (a plus!).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-directed, empathetic, and organized approach.
Critical thinking and outstanding interpersonal skills.
Leadership skills.

This is a hybrid role requiring in-home patient visits.
All candidates will complete a computer literacy and competency assessment prior to an in-person interview.
Starting pay is commensurate with relevant experience.
