What You'll Do
As a Home Health Occupational Therapist, you will partner with a Case Manager to deliver skilled therapy services in patients' homes. Your role will focus on helping individuals maintain their independence and safety by providing:
Home safety evaluations
Medication management strategies
Wheelchair seating and positioning solutions
Pain management techniques
Fall prevention programs
Chronic disease and dementia care support
Family caregiver training
Advocacy and patient education
Active participation in an interdisciplinary care team
What We're Looking For
Current, unrestricted California Occupational Therapist License
Current BLS/CPR Certification
Valid California Driver's License
Familiarity with local community resources
Understanding of illness/injury effects on daily living and the role of culture in healthcare
Knowledge of state and federal home care regulatory guidelines preferred
Experience in home health care is highly preferred
Experience with high-acuity patients is a plus
Strong case management skills and interdisciplinary teamwork experience are highly desired
Self-motivated, empathetic, and a critical thinker
Excellent interpersonal, leadership, and organizational skills
Pre-Interview Requirements
